丙泊酚预处理对兔心肌缺血再灌注损伤能量代谢的影响(英文)

摘    要:目的观察丙泊酚预处理对兔心肌缺血再灌注损伤(MIRI)能量代谢的影响。方法实验用新西兰雄性大白兔40只,随机分为4组(n=10):假手术组(Sham组)、缺血再灌注组(IR组)、缺血预处理组(IP组)和丙泊酚预处理组(PP组)。采用高效液相色谱(HPLC)法测定心肌组织中高能磷酸化合物三磷酸腺苷(ATP)、二磷酸腺苷(ADP)、一磷酸腺苷(AMP)和总腺苷酸量(TAN)、能荷(EC)的含量。检测心肌组织Na+/K+-ATPase,Ca^2+/Mg2+-ATPaSe和血清乳酸脱氢酶(LDH)及肌酸激酶(CK)的活性。结果IP组和PP组较IR组的ATP、ADP、AMP、TAN和EC含量明显增加,ATPase活性显著升高,LDH、CK-MB含量明显降低(P〈0.05,P〈0.01)。而IP组与PP组比较,各指标差异无统计学意义(P〉0.05)。结论丙泊酚预处理同缺血预处理一样可改善缺血再灌注损伤时的心肌能量代谢,其作用机制可能与增加缺血区的心肌能量储备。保护ATPase活性,改善能量代谢障碍有关。

Abstract:Objective] To study the effect of propofol preconditioning on energy metabolism in rabbits with my- ocardial ischemia-reperfusion injury. Methods] Forty New Zealand white rabbits 40 male, weighing (2.5±0.36) kg] were randomly assigned to 4 groups: sham operation group, ischemia reperfusion (IR) group, IR + ischemia precondi- tioning (IP) group and IR + propofol preconditioning (PP) group (n=10 each). The myocardial ischemia-reperfusion model of rabbits was established by coronary artery ligation. The high energy phosphates (ATP, ADP and AMP), total adenylic acid number (TAN), energy charge (EC) levels in myocardial tissue were measured by HPLC. The activities of serum lactate dehydrogenase (LDH), serum creatine kinase (CK) and ATPase in myocardial tissue were assayed. Results] The contents of ATP, ADP, AMP, TAN, EC and the activity of Na+/K+- ATPase and Ca2+/Mg2+-ATPase in PP group and IP group were higher than those in IR group, the contents of LDH and CK-MB in PP group and IP group were obviously lower than those in IR group in injured myocardial tissue (P<0.05, P<0.01), but when the IP group was compared with the PP group, the difference concerning the above contents was of no statistical vale (P> 0.005). All of the above results were of no difference in PP group and IP group (P>0.05). Conclusion] Propofol pre-conditioning as well as ischemia preconditioning protect the myocardial tissues from the iachemia-repeffusion injury by boosting ATPase activities associated with energy metabolism.
