精神分裂症患者恢复期自我概念的测评及健康教育的意义

摘    要:目的 探讨恢复期精神分裂症患者自我概念的状态水平及健康教育在康复中的作用.方法 采用父母教养方式评价量表(EMBU)、家庭环境量表(中文版FES-CV)对100例恢复期精神分裂症患者(研究组)和100名健康志愿者(对照组)进行比较,并对研究组患者在健康教育前后进行住院精神病人康复疗效量表(IPROS)和自我概念量表(TSCS)的测评.结果 两组比较,患者家庭表现为低亲密度、低情感表达、低成功性、低组织性以及高矛盾性和控制性差(P<0.05或P<0.01);在教养方式上,患者父亲表现为低情感温暖、高惩罚、严厉、过分干涉和拒绝否认(P<0.05),而母亲表现为低情感温暖、高拒绝否认、惩罚、严厉和偏爱被试(P<0.05或P<0.01,P<0.01);研究组健康教育前后TSCS、IPROS评分,有显著性差异(P<0.05).结论 家庭环境、父母的不良教养方式可能导致精神分裂症患者的个性缺陷,对其发病起重要作用;精神分裂症患者自我概念水平低下,健康教育能增强其信心,提高自我概念水平,改善临床疗效.

Abstract:Objective To understand the level and influential factors of self -concept in rehabilitated patients with schizophrenia and explore the effect of health education on them. Methods 100 eases of convales-cent patients with schizophrenia (study group) and 100 healthy volunteers (control group) were rated with egma minnen av bardndosna uppforstran (EMBU) and family environment scale (Chinese version of FES -CV) . Study group rated with inpatient psychotic rehabilitation observe scale (IPROS) and Tennessee serf -concept scale (TSCS) before and after health education. Results Patients with schizophrenia in study group had lower family cohesion performance, lower emotional expression, the lower success, lower and higher organizational conflicts and poor control than those in control group (P <0. 05 or P <0. 01) ;also lower emotional warmth and higher re-fuse and deny in father's rearing way (P < 0. 05), the lower warm feelings, higher refuse and deny, higher pref-erence than those in control group (P < 0. 05 or P < 0. 01) . There were significantly different in inpatient psychot-ic rehabilitation observe scale (IPROS) and Tennessee self-concept scale (TSCS) before and after health educa-tion. Conclusions The family environment, no -wellbeing style of parent's education might lead to personality defects in patients with schizophrenia which could play an important role in schizophrenia. Health education help the patients eliminate the inferiority complex and reinforce the confidence improve the self-concept and better the clinical effect.
