血管回声跟踪技术对正常高值血压伴高脂血症患者颈动脉弹性的研究

摘    要:目的:探讨血管回声跟踪(Echo tracking,ET)技术检测正常高值血压伴高脂血症患者颈总动脉弹性功能的变化及临床意义。方法:应用ET技术检测51例正常高值血压伴高脂血症患者(Ⅰ组)、62例正常高值血压患者(Ⅱ组)及50例正常人(Ⅲ组)双侧颈总动脉的弹性参数,即弹性系数(β)、僵硬度(Ep)、顺应性(AC)、增大指数(AI)、单点脉搏波传导速度(PWVβ),并用常规血管超声测量颈动脉内中膜厚度(IMT),进行对比分析。上述各组常规血管超声扫查未见明显异常。结果:各组左、右侧颈总动脉的各弹性指标比较均无统计学意义(P>0.05);Ⅰ组、Ⅱ组颈总动脉β较Ⅲ组明显增高,AC较Ⅲ组明显降低,Ⅰ组与Ⅱ组间的β、AC无明显差异;3组间颈总动脉的Ep、PWVβ比较均有统计学意义(P<0.05),AI、IMT比较均无统计学意义(P>0.05)。结论:正常高值血压伴高脂血症患者颈总动脉弹性明显降低;应用ET技术优于常规超声,能更早的发现颈总动脉弹性改变,为临床及时预防心脑血管疾病的发生提供依据。

Abstract:Objective: To investigate the carotid arterial elasticity in patients with high-normal blood pressure and hyperlipidemia by echo tracking technique.Methods: We selected 51 patients with highnormal blood pressure and hyperlipidemia,and 62 patients with high-normal blood pressure.Another 50 healthy cases were selected as normal control.Echo tracking technique was used to evaluate the carotid elastic modulus,including the stiffness parameter(β),pressure-strain elastic modulus(Ep),arterial compliance(AC),augment index(AI) and pulse wave conducting velocity(PWVβ).The intima-media thickness(IMT) was measured on routine ultrasonography.Carotid artery of the three groups showed no abnormalities on routine ultrasonography.Results: There was no difference in carotid elasticity between left and right side.Compared with the normal control group,β increased conspicuously in high-normal blood pressure and hyperlipidemia group and the high-normal blood pressure group,while AC was significantly lower.There was no difference in β and AC between the high-normal blood pressure and hyperlipidemia group and the high-normal blood pressure group.There was significant difference in Ep and PWVβ among the three groups,while there was no difference in AI and IMT.Conclusions: The elasticity of the carotid artery in high-normal blood pressure and hyperlipidemia group decreases obviously.Echo tracking technique may find the early change of the arterial elasticity,and it is superior than routine ultrasonography and may be helpful to prevent cardiac and cerebrovascular disease.
