溶栓治疗高血压并发心肌梗死对左室左房重构及功能变化的研究

摘    要:目的 研究溶栓治疗高血压并发心肌梗死对左室左房重构及功能变化的意义。方法 对69例高血压并发心肌梗死患者(分为溶栓组36例与非溶栓组33例)治疗前后应用超声心动图测量心脏结构和功能各项指标,比较溶栓组与非溶栓组心梗治疗后各项指标:左室舒张末期内径(LVDd)、室间隔与左室后壁舒张末期厚度(IVSTd和LVPWTd)、左室重量指数(LVMI)、左室射血分数(EF)、等容舒张时间(IRT)、左房内径(LAD)、左房面积(LAA)、左房射血分数(LAEF)、二尖瓣瓣尖处测量舒张早期峰值速度(E峰)及舒张晚期峰值速度(A峰)、E/A比值。结果和结论 溶栓组心梗治疗后上述各项指标较非溶栓组明显好转,溶栓治疗高血压并发心肌梗死对防止左室左房重构及其功能变化具有一定的临床意义。

Abstract:Objective Evaluating the changes of left ventricular and left Atrium structure and function in hypertensive patients acquired thrombolytic therapy afte acute myocardial infarction.Methods Echocardiography was applied to observe 69 patients in hypertension before and after acute myocardial infarction.These patients are divided into intravenous thrombolytic group (36 cases)and nonthrombolytic group (33 cases).Before and after myocardial infarction the parameters of LV end-diastolic diameter (LVDd)?end-diastolic septal thickness(IVSTd)?LV end-diastolic posterior wall thickmess(LVPWTd)?LN mass index(LVMi)?LV ejection fraction(EF)?left atrial diameter(LAD)?left atrial area(LAA)?left atrial ejection fraction(LAEF)?isovolumic relaxation time (IRT)?mitral valve diastolic E and A maximum velocity (E end A peak)?ratio of E/A were measured.Results All of the parameters in the thrombolytic group were better than in the thrombolytic group.
