临床药师从病原学和药物特点谈中西药结合抗感染思路

目的:通过临床药师走进病房对疑难病例实施个体化给药、优化治疗方案的过程,探讨临床药师走入临床的切入点及发挥的作用。方法:对4个具体病例与临床密切配合,根据患者病理生理情况综合分析可能的致病菌,结合相关指南和专家共识,依据药物的特点,为临床提供药学信息及优化治疗方案。结果:对患者住院期间的药物治疗效果评估后及时调整治疗方案,最终获得良好疗效。结论:临床药师通过药学专业知识优势,提出合理建议,中西药结合优化治疗方案,可以提高临床治愈率。

Thoughts of Clinical Pharmacists on Anti-infection by Using Traditional Chinese Medicine and Western Drugs from the Perspective of Etiology and Drugs Characteristics

OBJECTIVE： To optimize therapeutic regimen and to investigate the point of penetration and role of clinical pharmacists participating in clinical treatment by clinical pharmacists providing individual medication for difficult cases. METHODS： Clinical pharmacist provided pharmacy information, optimized treatment plan on four particular cases and analyzed pathogenic bacteria according to the pathological physiology combined with relevant guidelines and expert consensus on the basis of drug characteristics. RESULTS： Through drug treatment assessment and timely adjustment for hospitalized patients, ultimately good curative effect had been obtained. CONCLUSION： Clinical pharmacists with professional pharmacy knowledge can put forward reasonable suggestions and optimized treatment plan by integrating traditional Chinese medicine and western drugs to improve the clinical cure rate.
