临床药师在心胸外科查房的工作体会

目的：探讨在心胸外科开展临床药师查房工作的必要性、重要性及具体方式.方法：临床药师在心胸外科进行药学查房,主要为临床医师提供用药指导及抗菌药物合理应用管理措施的监管、使用华法林患者的用药教育及对护士进行药学知识的培训等内容.结果：临床药师通过在心胸外科的查房工作,提高了心胸外科围术期抗菌药物使用的规范性与合理性,并实时为临床医师提供用药指导,提高了医院合理用药水平.结论：临床药师参与外科查房工作效果显著,提高了临床医师合理用药水平及患者用药知识的宣教度,为促进医院药学发展提供了有力保障.

OBJECTIVE：To discuss the necessity and importance as well as the concrete modes of ward round in cardiothoracic surgery department participated by clinical pharmacists. METHODS： During ward round in cardiothoracic surgery department, clinical pharmacists provided medication guidance for clinicians, monitoring the management measures on rational use of antibiotics, carried out medication education among patients receiving warfarin and training among nurses about pharmaceutical knowledge etc. RESULTS： Clinical pharmacists＇ participation in the ward round in cardiothoracic surgery department promoted the standardization and rationality of perioperative antibiotic use, provided timely medication guidance for clinicians and improved the level of rational use of drugs. CONCLUSION： Clinical pharmacists＇ ward round in cardiothoracic surgery department achieved noticeable outcome, improved clinicians＇ level of rational use of drugs and enhanced the propaganda on medication knowledge among patients and provided strong guarantee for the development of hospital pharmacy.
