乳腺癌患者外周血EMA、EGP-2和CK19的检测及其临床意义

摘    要:目的检测乳腺癌患者外周血上皮膜抗原(EMA)、表皮糖蛋白基因(EGP-2)和细胞角蛋白19(CK19)的表达,探讨EMA、EGP-2和CK19作为乳腺癌微转移指标的应用价值。 方法应用流式细胞术检测65例乳腺癌患者和37例乳腺良性疾病患者外周血EMA 、EGP-2和 CK19的表达情况,分析其与TNM分期、年龄、肿瘤大小、腋窝淋巴结转移以及ER和PR表达状态等临床病理学指标的关系。结果乳腺癌患者外周血EMA、EGP-2和CK19的阳性表达率均高于对照组(P<0.05);其阳性细胞表达率与TNM分期、肿瘤大小、腋窝淋巴结转移情况明显相关(P<0.05)。3指标联合检测相对于2项或单项检测敏感度及准确性升高(P<0.05)。 结论联合检测EMA、EGP-2和CK19可提高诊断正确率,有可能在乳腺癌预后预测和治疗中发挥作用。

Abstract:Objective To explore the expressions of EMA,EGP-2 and CK19 in peripheral cells of breast cancer and their diagnostic value as micro-metastasis targets for breast cancer.Methods EMA,EGP-2 and CK19 were determined by flow cytometry in 65 cases of breast cancer and 37 cases of benign breast diseases,also the TNM stages,age,size of tumors,maxillary lymph nodes metastasis and expressions of estrogen receptors and progestin receptors were analyzed.Results The expression rates of EMA,EGP-2 and CK19 in peripheral cells were higher than those in the normal controls(P<0.05) and closely related to the TNM stages,size of tumor and maxillary lymph node metastasis(P<0.05).The sensitivity and accuracy of the EMA,EGP-2 and CK19 combination were higher than those of one or two(P<0.05).Conclusions Combination detection of EMA,EGP-2 and CK19 improves the diagnostic sensitivity and accuracy,and may play an important role in the prognosis and treatment of breast cancer.
