老年高血压患者血压变异性与动脉粥样硬化的相关性研究

摘    要:目的研究老年高血压患者血压变异性与动脉硬化之间的关系。方法 126例老年高血压患者,按有无合并冠状动脉粥样硬化性心脏病分为两组。测定臂踝脉搏波传导速度,并采用无创血压监测仪行24h血压监测,分析高血压患者24h、白天及夜间收缩压和舒张压变异性。结果 EH+CHD组臂踝脉搏波传导速度高于EH岁组(P<0.05);EH+CHD组日间收缩压标准差(dSBP-SD)、24h收缩压标准差(24hSBP-SD)均显著高于EH组(P<0.01或0.05);24hSBP-SD、DSBP-SD与PWV呈显著正相关。结论血压变异性与动脉粥样硬化、动脉扩张性有着密切的关系,控制血压、降低血压变异性对延缓动脉粥样硬化进展起到了重要作用。

Abstract:Objective To investigate the relationship between blood pressure variability and the arterial distensibility in elderly patients with essential hypertension. Methods A total of 126 elderly hypertensive patients were divided into two groups. Auto-survey atherosclerosis apparatus was applied to examine brachial-ankle pulse wave velocity (baPWV). Ambulatory blood pressure monitoring was examined to calculate the systolic and diastolic BP variability. Results BaPWV were significantly higher in EH+CHD group than in EH group (P〈0.05). DSBP-SD and 24bSBP-SD in EH patients with CHD group were significantly higher than those in EH patients without CHD group (P〈0.05 or P〈0.01). Pearson correlation analysis showed that 24 hSBP-SD, DSBP-SD were significantly related to PWV. Conclusion There was positive association between blood pressure variability and arterial distensibility in hypertension patients.
