中西医结合介入法治疗股骨头坏死的临床效果观察

目的探讨分析中西医结合介入法治疗股骨头坏死的疗效。方法选取郑州市第二人民医院自2008年10月到2010年9月收治的49名股骨头缺血性坏死患者,49名患者均经临床与影像学确诊。治疗组采用Seldinger改良穿刺技术以及活血化瘀的中药进行治疗,缓慢注入罂粟碱30mg,30mL丹参注射液,240mg川芎嗪,术后当天起静脉滴注5%的葡萄糖液+30mL丹参注射液;对照组注入30mg罂粟碱,50万U尿激酶,50mL低分子右旋糖酐。术后当日用静脉滴注5%葡萄糖液+6万U尿激酶,250mL低分子右旋糖酐,之后每天继续静脉滴注;两组均持续5d。结果经中西医结合介入治疗股骨头坏死,1个疗程后,患者症状明显有所缓解。3个月之后,进行随访观察发现,观察组25例病患,治愈12例,显效7例,好转4例,无效2例,有效率为92.0%,对照组24例病患,治愈4例,显效9例,好转6例,无效6例,有效率为76.0%,观察组的治疗效果明显高于对照组,对比具有显著性差异,具有统计学意义(P<0.05)。结论中西医结合介入治疗股骨头缺血坏死可以有效改善股骨头的供血,缓解患者的症状,效果明显。

Clinical Efficacy Observation of Integrated Traditional Chinese and Western Medicine united with Interventional Surgery in Treating Femoral Head Necrosis

Object To investigate the clinical efficacy of integrated traditional Chinese and western medicine（TCM-WM） united with interventional surgery in treating femoral head necrosis.Methods Select 49 cases of patients diagnosed as femoral head necrosis during Oct 2008 to Sep 2010 in Zhengzhou Second People＇s Hospital as observers.And put them into two groups as therapy group and control group.The therapy group underwent Seldinger improved puncture with medicine activating blood circulation,30mg narceine,30ml injection of red sage root,240mg ligustrazine were injected slowly during surgery.Then 5% glucose injection and 30ml injection of red sage root were injected via vein post-surgery.The control group injected 30mg narceine,0.5 million IU Urokinase and 50ml dextran-40.Then 5% glucose injection,0.6 million IU Urokinase and 250ml dextran-40 were injected via vein with 5days post-surgery.Results The clinical symptoms were obviously eased after one course of integrated traditional Chinese and western medicine united with interventional surgery.The observation 3 months later suggested that there were 12 of 25 in observe group were cured,7 of 25 were obvious efficacy,and 4 of 25 were improved,thus the total efficacy of observe group was 92%,while that of control group was 76%.The efficacy of observe group was obviously better than control group（P〈0.05）.Conclusion The integrated traditional Chinese and western medicine（TCM-WM） can obviously improve the blood supply of femoral head necrosis,and eased the patients＇ clinical symptoms.
