老年髋部骨折病人不同危险因素对围术期发生肺部感染的影响

目的:探讨老年髋部骨折病人合并不同危险因素对围术期并发肺部感染的影响。方法:回顾性分析120例老年髋部骨折病人的临床资料，其中围术期并发肺部感染者60例，无肺部感染者60例，比较2组病人围术期的临床指标，应用logistic回归分析老年髋部骨折病人围术期并发肺部感染的危险因素。结果:单因素分析结果显示，吸烟、慢性阻塞性肺疾病（COPD）史、糖尿病史、心脏病史、机械通气、输注红细胞、术前低体质量指数、术前低白蛋白、入住ICU、手术时间以及住院时间是影响老年髋部骨折病人围术期并发肺部感染的危险因素（P<0.05~P<0.01）；多因素回归分析显示，年龄>80岁、COPD史、机械通气、糖尿病史、手术时间≥ 2 h为影响老年髋部骨折病人围术期并发肺部感染的独立危险因素（P<0.05）。结论:老年髋部骨折病人年龄>80岁、COPD史、糖尿病史、进行机械通气及手术时间≥ 2 h会增加病人围术期并发肺部感染的风险。

Effect of different risk factors on perioperative pulmonary infection in elderly hip fracture patients

Objective: To investigate the effects of different risk factors on perioperative pulmonary infection in elderly hip fracture patients.Methods: The clinical data of 120 elderly patients with hip fracture,which included 60 cases complicated with pulmonary infection and 60 cases complicated without pulmonary infection,were retrospectively analyzed.The clinical indexes between two groups were compared.The risk factors of hip fracture complicated with perioperative pulmonary infection in elderly patients were analyzed using logistic regression analysis.Results: The results of univariate analysis showed that the smoking,chronic obstructive pulmonary disease,history of diabetes,heart disease,mechanical ventilation,transfusing red bloodcell,preoperative low body mass index,preoperative low albumin,ICU admission,operation time and hospital stay were the risk factors of perioperative pulmonary infection in elderly patients with hip fracture(P<0.05 to P<0.01).The results of multivariate regression analysis showed that the age more than 80 years,chronic obstructive pulmonary disease,history of diabetes,mechanical ventilation and operation time ≥ 2 h were the independent risk factors of perioperative pulmonary infection in elderly patients with hip fracture(P<0.05).Conclusions: The age more than 80 years,chronic obstructive pulmonary disease,history of diabetes,mechanical ventilation and operation time ≥ 2 h can increase the risk of perioperative pulmonary infection in elderly patients with hip fracture.
