HP感染与胃粘膜肠化中CDX2蛋白表达的关系

摘    要:目的 探讨HP感染与胃粘膜肠化中CDX2蛋白表达的关系。方法 正常的胃粘膜8例、浅表性胃炎11例、萎缩性胃炎18例、萎缩性胃炎伴肠上皮化生30例,应用Warthin-Starry硝酸银染色检测HP,应用免疫组织化学SP法检测CDX2蛋白。结果 8例正常胃粘膜内未检测出HP,CDX2蛋白无表达。HP在萎缩性胃炎组和肠上皮化生组的阳性率分别为77.78%、63.33%,显著高于浅表性胃炎组18.18%(P〈0.01,P〈0.05)。CDX2蛋白在浅表性胃炎组无表达,在肠上皮化生组的阳性率为83.33%,显著高于萎缩性胃炎组(P〈0.01)。CDX2蛋白在肠上皮化生组HP阳性者中的表达显著高于HP阴性者(P〈0.01)。结论 HP感染、CDX2蛋白异常表达与胃粘膜肠化有关,HP引起胃粘膜肠化的途径之一可能是启动CDX2基因的异常表达。

Abstract:Objective To investigate the relationship between Helicobacter pylori (HP) infection and the expression of CDX2 in Intestinal Metaplasia (IM) of stomach.Methods 8 cases of normal gastric mucosa, 11 cases of chronic superficial gastritis(CSG), 18 cases of chronic atrophic gastritis (CAG) and 30 cases of IM were available chose. HP infection was detected using Warthin-Starry bacterium staining method. The expression of CDX2 was studied with immunohistochemical streptavidin-peroxidase method. Results There was no HP infection and no expression of CDX2 in normal gastric mucosa. The positive rate of HP in the CAG group(77.78%) and IM group (63.33%) were both higher than that in the CSG group(18.18%)(P<0.01, P<0.05 respectively). No expression of CDX2 was observed in CSG. The positive rate of CDX2 in the IM group was 83.33%, which was higher than that in the CAG group(P<0.01).Moreover, in the IM group, the positive rate of CDX2 in positive HP group was higher than that in negative HP group(P<0.01). Conclusions IM is related with both HP infection and ectopic expression of CDX2. HP may switch on CDX2 gene, which may trigger the initiation and development of IM in the stomach.
