LASIK手术角膜瓣蒂不同位置的疗效对比

目的对角膜瓣蒂位于上方与鼻侧的准分子激光原位角膜磨镶术(LASIK)治疗近视的效果进行比较。方法随机选用旋转式角膜刀制作上方蒂组(A组)LASIK350例(678只眼)和平推式角膜刀制作鼻侧蒂组(B组)LASIK170例(324只眼),对两组患眼术后视力、屈光度、术中及术后早期并发症进行比较。结果术后1年裸眼视力A组和B组分别为1.03±0.12和1.00±0.12,两组比较无明显差异(P>0.05);术后1年剩余屈光度两组比较无显著性差异;与制作角膜瓣有关的并发症发生率A组低于B组,切缘出血A组明显减少(P<0.05)。结论A组与B组均具有良好的术后效果,随访1年两组疗效无明显差异,A组角膜瓣稳定性优于B组。

Comparison of Effects of LASIK with Upper and Nasal Corneal Flaps

Objective To evaluate the effects of laser in situ keratomileusis(LASIK)with upper and nasal pedicle of corneal flaps on the correction of myopia.Methods According to randomized fashion,LASIK was performed on 678 eyes of 350 cases with upper pedicle of corneal flap using rotating microkeratome(group A)and 324 eyes of 170 cases with nasal pedicleof corneal flap using oscillating microkeratome(group B).The effects of their postoperative naked visual acuity,diopter and early postoperative complications were compared between two groups based on the pedicle position of corneal flaps.Results The uncorrected postoperative visual acuities of groups A and B were(1.03 0.12)and(1.0 0.12),respectively.After 1 year,there was no significant difference between them(P>0.05),there was no significant difference of the diopter between them.For group A,the incidence of complications which related to corneal flaps was lower,and the rate of bleeding in the cut edge was also more lower(P<0.05).Conclusions LASIK operations in groups A and B are both effective,and there is no significant difference between them in two year follow-up period.However,the group A is superior to the group B on the visual recovery,which has the operative stability of corneal flap and almost no bleeding in the cut edge.
