摘    要:目的:探讨近5年我院常见病原菌对喹诺酮类药物的耐药性变迁。方法:采用纸片扩散法进行药敏试验,根据美国临床实验室标准化研究所(CLSI)文件标准判断结果;采用WHONET-5软件对耐药性数据进行分析。结果:2003~2007年产超广谱β-内酰胺酶(ESBLs)大肠埃希菌和克雷伯菌的检出率分别为43.6%~60.1%和51.0%~65.1%,耐甲氧西林金黄色葡萄球菌(MRSA)和耐甲氧西林凝固酶阴性葡萄球菌(MRSCN)检出率分别为50.8%~83.3%和80.0%~81.5%,临床常见病原菌对喹诺酮类药物的耐药性呈逐年上升趋势,尤以MRSA、MRSCN、屎肠球菌、粪肠球菌、大肠埃希菌对氟喹诺酮类耐药更为明显。结论:应加强临床常见病原菌对喹诺酮类药物的细菌耐药监测,了解其耐药变迁,指导临床合理用药,防止耐药菌株的传播。

Abstract:OBJECTIVE: To investigate the change of the resistance of common pathogenic bacteria isolated in the First Clinical Hospital of Jilin University during the past 5 years to quinolones. METHODS: Disk diffusion test was used to study the susceptibility test, with the results evaluated based on US Clinical and Laboratory Standards Institute(CLSI) . WHONET-5 software was used to analyze the drug resistance data. RESULTS: During 2003-2007, the detection rates of extended spectrum beta- lactamases(ESBLs) -producing Escherichia coli and Klebsiella.spp were 43.6%- 60.1% and 51.0%- 65.1%, respectively; the detection rates of methicillin- resistant Staphylococcus aureus(MRSA) and methicillin- resistant coagulase negative Staphylococcus(MRSCN)were 50.8%- 83.3% and 80.0%- 81.5%, respectively; the resistance of common pathogenic bacteria to quinolones showed an increasing tendency, much as in MRSA, MRSCN, feces enterococci, Enterococcus faecalis, and Escherichia coli. CONCLUSION: Monitoring of the resistance of common pathogenic bacteria to quinolones should be strengthened. The change of the antimicrobial resistance should be investigated in order to direct rational drug use in the clinic and prevent the propagation of drug - resistant strains.
