鼻咽癌肌肉损害的MRI诊断

目的:着重研究鼻咽癌肌肉损害MRI表现特征及其临床价值。方法:搜集经临床和病理确诊为鼻咽癌伴有头颈部肌肉损害MRI资料78例,进行其MRI表现特征及相关病理改变研究,以提高诊断水平。结果:鼻咽癌伴头颈部肌肉损害的MRI表现如下:部位:腭帆提肌和腭帆张肌73例,翼外肌42例,翼内肌16例,咽缩肌43例,头长肌67例,颈长肌21例,咬肌36例,胸锁乳突肌23例,二腹肌后腹51例,颞肌3例,舌肌7例,常见多肌肉损害,单发少见。形态:肌肉外形肿胀35例,肌肉变形移位26例,变性萎缩17例。信号:T1加权呈低信号、T2加权呈高信号41例,压脂后呈高低不均信号37例。增强:病灶呈明显强化32例,不均强化21例,环状强化5例,无强化20例。其他:颅底骨质破坏29例,海绵窦侵犯23例,颈静脉孔破坏15例,舌下神经管破坏7例。结论:MRI检查对鼻咽癌伴头颈部肌肉损害的部位、范围,以及原发或继发改变的显示及病理基础等,具有重要诊断价值。

MRI Scanning of Nasopharyngeal Carcinoma Damaging Muscle

Objective:To study MRI features and its clinical value of muscular damage from nasopharyngeal carcinoma(NPC).Methods:MRI features and pathologic basis of 78 patients with muscular damages of head and neck invaded by NPC confirmed by clinical and histological manifestations were evaluated,respectively.Results:MRI findings in 78 cases of NPC as following:site:73 cases in musculus levator veli and musculus tensor veli palatine,42 cases in musculus pterygoideus lateralis,16 cases in musculur pterygoideus medialis,43 cases in musculus constrictor pharyngingis,67 cases in musculus longus capitis,21 cases in musculus longus colli,36 cases in musculas masseter,23 cases in musculus stero-cleidomastoideus,51 cases in venter posterior musucala digastrici,3 cases in temparal muscules,and 7 cases in musculi linguae,Appearance:Thirty-five cases in the form of muscular swelling,26 cases in displacement and deformation of muscles,and 17 cases in degeneration and atrophy of muscles.Signal:41 cases in low signal of T1WI and high signal of T2WI,37 cases in hetergeneity of signal.Gd-DTPA enhancement:32 cases in obvious enhanced muscular damage,21 cases in hetergentiy with no enhancement.5 cases in circular enhancement,2.cases enhanced,Other:29 cases in cranial base destructed,23 cases in cavenous sinus invaded,15cases in jugular foramen destruction,7 cases in hypoglossi canalis destruction.Conclusion:The MRI examination had its important value in the diagnosis of the position,extension and primary or second changes in the muscles damaged by NPC.
