阵发室上性心动过速时胸痛合并ST段压低对冠心病的诊断价值

目的确定阵发性室上性心动过速（PSVT）发作时胸痛合并心电图ST段压低的冠心病（CHD）发生概率。方法对仅在PSVT发作时发生胸痛或ST段压低而行冠状动脉造影者，收集其临床资料及冠状动脉造影结果进行分析。共58例患者纳入，其中单纯胸痛患者22例，单纯ST段压低者14例，既有胸痛又有显著ST段压低者22例。结果冠脉造影狭窄例数：单纯胸痛组0例；单纯ST段压低组1例；胸痛合并ST段压低组3例。该组CHD发生率高于其它两组，但发生概率仍较低，仅占13．6％。结论PSVT发作时出现胸痛合并ST段压低，其CHP发生率高于单纯胸痛或单纯ST段压低者，但发生概率较低，对该类患者是否应在射频消融术前行冠状动脉造影仍需进一步评价。

Prevalence of coronary heart disease in patients with chest pain and ST segment depression during supraventricular tachycardia episodes

Objective To ascertain the prevalence of coronary heart disease （CHD） in patients experience typical chest pain and ST-segment depressions on electrocardiograms during episodes of paroxysmal supraventricular tachycardia （PSVT）. Methods Patients with PSVT undergoing coronary angiography were evaluated. In these patients, there are 22 patients suffering from chest pain without ST-segment depression （Group 1）, 14 patients having ST- segment depressions without chest pain （Group 2） ,and 22 patients both having chest pain and ST segment depressions at the same time （Group 3）. Results CHD was diagnosed in 0 patient in Group 1, 1 patient in Group 2, and 3 patients in Group 3. Conclusion Patients with chest pain and ST-segment depressions during episodes of PSVT have a low CHD prevalence rate. Whether performing coronary angiography in these patients before radiofrequency ablation should be further evaluated.
