重型颅脑损伤早期瞳孔变化与预后关系的量化研究

目的探讨重型颅脑损伤患者早期瞳孔变化评分与预后的关系。方法观察106例重型颅脑损伤患者伤后6h内的瞳孔变化并对其量化评分,依据伤后3个月格拉斯哥结果分级(GOS)标准分为预后良好组和预后不良组,分析瞳孔变化评分与GCS评分和GOS评分的关系。结果预后良好组患者的瞳孔变化评分显著低于预后不良组(P<0.01),瞳孔变化评分与GCS评分负相关(r=-0.772,P<0.01)、与GOS评分负相关(r=-0.820,P<0.01)。结论伤后早期的瞳孔变化评分可反映颅脑损伤的严重程度,评分越高,预后越差。

Quantitative analysis of the correlation between the early status of pupils and the prognosis of patients with severe head injuries

Objective To study the correlation between the early status of pupils and the prognosis of patients with severe head injuries. Methods The status of pupils within the first 6 hours of injury from 106 patients were observed and scored. Patients were divided into good outcome group and bad outcome group according to Glasgow Outcome Scale (GOS) at 3 months postinjury. The pupils status score, GCS and GOS were retrospectively analyzed. Results Analysis revealed that pupils status score of patients in good outcome group was significantly lower than that in bad outcome group (P<0.01). And which was significantly negatively correlated with GCS and GOS score (r=-0.772, P<0.01; r=-0.820, P<0.01, respectively). Conclusions The pupils status score within the first 6 hours of injury, similar to GCS score, can reflect the severity degree of head injury. The higher the score, the worse the prognosis.
