结核性腹膜炎的64排螺旋CT平扫及增强特征分析

目的分析结核性腹膜炎(TBP)的64排螺旋CT平扫及增强特征。方法回顾性分析2018年1月至2019年1月我院收治的30例TBP患者的腹部CT图像,分析CT平扫及增强表现为腹水、腹膜、大网膜、肠系膜异常及腹腔淋巴结肿大的情况。结果经CT扫描发现,30例患者中存在腹水20例,其中少量积液12例,包裹性积液8例,CT值范围10~30 HU。12例检出腹膜增厚,其中8例壁腹膜增厚,2例结节样不规则增厚,2例合并腹膜结核瘤。大网膜改变共计6例,其中4例呈污垢样增厚、粘连,1例呈饼状增厚,1例呈结节状增厚。肠系膜增厚检出3例。腹腔淋巴结肿大检出4例。结论TBP患者的常见CT特征包括腹水、肠系膜增厚、大网膜受累的污垢样增厚、腹膜光滑的均匀增厚及腹腔淋巴结肿大。

Analysis of plain and enhanced characteristics of 64-row spiral CT in tuberculous peritonitis

Objective To analyze the plain and enhanced characteristics of 64-row spiral CT in tuberculous peritonitis(TBP).Methods The abdominal CT images of 30 patients with TBP admitted in our hospital from January 2018 to January 2019 were analyzed retrospectively.Ascites,abnormal peritoneum,omentum majus,mesentery and abdominal lymphadenopathy showed by the plain and enhanced CT images were analyzed.Results By CT scan,20 cases of ascites were found in 30 patients,including 12 cases of small amount of effusion,8 cases of encapsulated liquid,with CT value ranging from 10 to 30 HU.Peritoneal thickening was detected in 12 cases,including 8 cases with wall peritoneal thickening,2 cases with irregular nodular thickening,and 2 cases with peritoneal tuberculoma.There were 6 cases of omentum majus changes,including 4 cases of dirt thickening and adhesion,1 case of pie thickening and 1 case of nodular thickening.Three cases of mesenteric thickening were detected,four cases of abdominal lymphadenopathy were detected.Conclusion The common CT characteristics of TBP include ascites,mesentery thickening,dirt thickening of omentum majus,smooth and even thickening of peritoneum,and abdominal lymphadenopathy.
