宫腹腔镜联合治疗子宫切口憩室的临床效果

目的分析宫腹腔镜联合治疗子宫切口憩室的临床效果。方法将60例剖宫产术后子宫切口憩室患者按数字随机表法分为阴式组(n=30,经阴道修补术)及宫腹腔镜组(n=30,宫腹腔镜联合术式),比较两组的治疗效果。结果宫腹腔镜组的手术时间长于阴式组,术中出血量少于阴式组,术后排气时间、术后阴道流血时间及住院时间短于阴式组(P<0.05)。两组的治疗总有效率、术后妊娠率无显著差异(P>0.05)。术后3个月,两组的月经时间均短于术前(P<0.05);术后3个月,两组的月经时间无显著差异(P>0.05)。宫腹腔镜组的并发症总发生率低于阴式组(P<0.05)。结论宫腹腔镜联合治疗子宫切口憩室能缩短患者术后恢复时间,临床应用价值高。

Clinical effect of hysteroscopy combined with laparoscopy in the treatment of uterine incision diverticulum

Objective To analyze the clinical effect of hysteroscopy combined with laparoscopy in the treatment of uterine incision diverticulum.Methods Sixty patients with uterine incision diverticulum after cesarean section were randomly divided into vaginal group(n=30,transvaginal repair)and hysteroscopy combined with laparoscopy group(n=30,hysteroscopy combined with laparoscopy operation)according to the randorn number table method.The treatment effects of the two groups were compared.Results The operation time of the hysteroscopy combined with laparoscopy group was longer than that of the vaginal group,and the intraoperative blood loss volume was less than that of the vaginal group,the postoperative exhaust time,postoperative vaginal bleeding time and hospital stay were shorter than those in the vaginal group(P<0.05).There were no significant differences in the total effective rate of treatment and postoperative pregnancy rate between the two groups(P>0.05).At 3 months after operation,the menstrual time was shorter in both groups than before operation(P<0.05);at 3 months after operation,there was no significant difference in menstrual time between the two groups(P>0.05).The total incidence of complications in the hysteroscopy combined with laparoscopy group was lower than that in the vaginal group(P<0.05).Conclusion Hysteroscopy combined with laparoscopy treatment of uterine incision diverticulum can shorten patients'postoperative recovery time and has high clinical application value.
