妊娠期糖尿病对新生儿早期并发症及血脂水平的影响

摘    要:
目的探究妊娠期糖尿病对新生儿早期并发症发生情况及血脂水平的影响。方法随机抽取2016年10月至2018年10月我院90例健康孕妇(对照组)及90例妊娠期糖尿病患者(试验组)为观察对象,比较两组新生儿并发症发生率及血脂水平。同时,结合90例妊娠期糖尿病患者血糖控制情况将其分为血糖控制满意组(61例)和血糖控制不满意组(29例),对比两组新生儿并发症发生率及血脂水平。结果试验组巨大儿、新生儿感染、窒息、低血糖、高胆红素症及呼吸窘迫综合征发生率均高于对照组(P<0.05)。试验组新生儿TC、TG、LDL-C水平均高于对照组,HDL-C水平低于对照组(P<0.05)。血糖控制满意组巨大儿、新生儿感染、低血糖及高胆红素症发生率均显著低于血糖控制不满意组(P<0.05),但两组新生儿窒息、呼吸窘迫综合征发生率无显著差异(P>0.05)。血糖控制满意组新生儿TG、LDL-C水平明显低于血糖控制不满意组(P<0.05),但两组TC、HDL-C水平比较无显著差异(P>0.05)。结论妊娠期糖尿病是导致巨大儿、新生儿感染、低血糖、高胆红素症、窒息、呼吸窘迫综合征等并发症以及血脂水平升高的高危因素,而孕期血糖控制效果良好有助于降低新生儿并发症发生率及血脂水平,保障母婴安全。

Abstract:
Objective To investigate the effects of gestational diabetes mellitus on early complications and blood lipid levels of newborn.Methods From October 2016 to October 2018,90 healthy pregnant women(control group)and 90 patients with gestational diabetes mellitus(experimental group)were randomly selected as observation objects to compare the incidence of neonatal early complications and blood lipid level of the two groups.At the same time,combining with the blood glucose control of 90 patients with gestational diabetes mellitus,the patients were divided into blood glucose control satisfaction group(61 cases)and blood glucose control dissatisfaction group(29 cases),and the incidence of neonatal early complications and the blood lipid levels were compared between the two groups.Results The incidences of macrosomia,neonatal infection,asphyxia,hypoglycemia,hyperbilirubinemia and respiratory distress syndrome in the experimental group were higher than those in the control group(P<0.05).The levels of TC,TG and LDL-C in the experimental group were higher than those in the control group,and the level of HDL-C was lower than that in the control group(P<0.05).The incidences of macrosomia,neonatal infection,hypoglycemia and hyperbilirubinemia in the blood glucose control satisfaction group were significantly lower than those in the blood glucose control dissatisfaction group(P<0.05),but there were no significant differences in the incidences of neonatal asphyxia and respiratory distress syndrome between the two groups(P>0.05).The levels of TG and LDL-C in the blood glucose control satisfaction group were significantly lower than those in the blood glucose control dissatisfaction group(P<0.05),but there were no significant differences in the levels of TC and HDL-C between the two groups(P>0.05).Conclusion Gestational diabetes mellitus is a high risk factor for neonatal complications such as macrosomia,neonatal infection,hypoglycemia,hyperbilirubinemia,asphyxia,respiratory distress syndrome and the increase of blood lipid level.The good effect of gestational blood glucose control is helpful to reduce the incidence of neonatal complications and blood lipid levels,and ensure the safety of mothers and infants.
