后路长节段与短节段椎弓根钉棒系统固定术治疗退变性脊柱侧弯的效果

目的比较后路长节段与短节段椎弓根钉棒系统固定术治疗退变性脊柱侧弯的效果。方法选取本院收治的64例退变性脊柱侧弯患者为研究对象,依据随机对照原则将其分为对照组和观察组,各32例。对照组行后路短节段椎弓根钉棒系统固定术,观察组行后路长节段椎弓根钉棒系统固定术。比较两组患者的治疗效果。结果对照组手术时间短于观察组,术中出血量少于观察组(P<0.05);术后1个月,两组的Cobb角均小于术前,且观察组小于对照组(P<0.05);术后,两组VAS、ODI评分均低于术前,且观察组低于对照组(P<0.05);对照组的术后并发症总发生率为6.25%,略低于观察组的15.62%,但差异无统计学意义(P>0.05)。结论后路长节段固定融合术利于Cobb角与脊柱功能恢复,而短节段固定融合术在手术时间、术中出血量以及术后并发症方面有明显优势。

Effect of long and short segment posterior fixation with vertebral pedicle screw system in the treatment of degenerative scoliosis

Objective To compare the effect of long and short segment posterior fixation with vertebral pedicle screw system in the treatment of degenerative scoliosis.Methods A total of 64 patients with degenerative scoliosis admitted in our hospital were selected as the study objects.The patients were divided into control group and observation group according to the principle of randomized control,with 32 cases in each group.The control group was treated with short segment posterior fixation with vertebral pedicle screw system,and the observation group was treated with long segment posterior fixation with vertebral pedicle screw system.The therapeutic effects of the two groups were compared.Results The operation time of the control group was shorter than that of the observation group,and the intraoperative blood loss volume was less than that of the observation group(P<0.05).At one month after operation,the Cobb angle in the two groups were smaller than those before operation,and that in the observation group was smaller than the control group(P<0.05).After the operation,the scores of VAS and ODI of the two groups were lower than those before operation,and those in the observation group were lower than the control group(P<0.05).The total incidence of postoperative complications in the control group was 6.25%,which was slightly lower than 15.62%in the observation group,but the difference was not statistically significant(P>0.05).Conclusion Long segment posterior fixation and fusion is beneficial to the recovery of Cobb angle and spinal function,while short segment fixation and fusion has obvious advantages in operation time,intraoperative blood loss volume and postoperative complications.
