经皮穴位电刺激对乳腺手术中瑞芬太尼使用剂量的影响

摘    要:
目的探讨经皮穴位电刺激对乳腺手术中瑞芬太尼使用剂量的影响。方法选取于我院行全麻下乳腺手术的200例患者,采用随机数字表法将其分为对照组和试验组,各100例。试验组于麻醉诱导前30 min至术毕,持续刺激双侧合谷-内关穴;对照组在穴位上贴电极,不进行穴位电刺激。比较两组患者的瑞芬太尼用量及拔管时间;比较两组患者术后24 h的疼痛程度及术后并发症发生情况。结果试验组的瑞芬太尼用量明显少于对照组,拔管时间明显短于对照组(P<0.05)。两组术后24 h的NRS评分及术后头晕、恶心呕吐发生率比较,差异均无统计学意义(P>0.05)。结论经皮穴位电刺激可减少乳腺手术中瑞芬太尼的用量,缩短拔管时间,有利于患者术后恢复。

Abstract:
Objective To investigate the influence of transcutaneous electrical acupoint stimulation on the using dosage of remifentanil in breast surgery.Methods A total of 200 patients who received breast surgery under general anesthesia in our hospital were selected.According to the random number table method,the patients were divided into control group and experimental group,with 100 cases in each group.In the experimental group,both Hegu and Neiguan acupoints were continuously stimulated from 30 minutes before anesthesia induction to the end of operation;in the control group,the electrodes were pasted on the acupoints without electrical stimulation.The dosage of remifentanil and extubation time were compared between the two groups;the pain degree of 24 hours after operation and postoperative complications were compared between the two groups.Results The dosage of remifentanil in the experimental group was significantly less than that in the control group,and the extubation time was significantly shorter than that in the control group(P<0.05).There were no significant differences in NRS score and incidences of postoperative dizziness,nausea and vomiting between the two groups(P>0.05).Conclusion Transcutaneous electrical acupoint stimulation can reduce the dosage of remifentanil in breast surgery,shorten the time of extubation,and help patients'recover after operation.
