老年心肺联合疾病院前急症发生特点分析

目的分析老年患者心肺联合疾病院前以急症发生特点和疾病谱的构成、相关因素及抢救疗效,探讨对此类急症发生的抢救、防治策略及意义。方法选择院前以急性呼吸困难为主诉并需呼叫120出救护车进行院前抢救的老年患者共568例,统计分析其特点、疾病构成、相关因素及随访治疗效果。结果心肺联合2种疾病急症400例(占70.42%),其中患者或家属自认为1种疾病的占73.25%;误诊、漏诊率13.56%。以慢性支气管炎肺气肿合并冠心病急性左心衰发作最常见,与年龄、吸烟等危险因素密切相关。结论心肺联合疾病急症的发生占急诊抢救病人的比例较大,应予高度重视。院前积极防治心肺联合疾病急症的发生具有重要意义。

Characteristics of the emergency of prehospital of united heart and lung disease in the elderly

Objective Analyzing the characteristics of the united emergencies of heart disease and pulmonary disease in elderly patients and investigating disease component, related factors, emergency treatment,prevention and treating strategy. Methods Five hundred and sixty-eight cases of elderly patients undergoing emergency treatment of prehospital on the ambulance for dyspnea were chosen. The characteristics, disease component, related factors and curative effects of the emergencies were analyzed. Results Of 568 cases, 400 suffered from both heart disease and pulmonary disease. 73.25% out of these 400 cases were reported by their family to have just heart disease or pulmonary disease.The misdiagnosis rate was 13.56%.The incidence rate of chronic bronchitis and pulmonary emphysema was highest, which might be related with age and smoking. Conclusions The incidence rate of the united emergencies of heart diseases and pulmonary diseases is high.It is of importance to prevent and treat the united emergencies heart disease and pulmonary disease.
