摘    要:目的 观察高通量与常规透析对维持性血液透析(MHD)患者钙磷代谢、甲状旁腺激素及血脂的影响,探讨高通量透析的临床效果.方法 选取40例MHD患者,按随机数字表法分为常规透析组(HD组)和高通量透析组(HFHD组),分别采集第1天和治疗3个月后透析前血标本,检测血钙、血磷、甲状旁腺激素(PTH)、甘油三酯(TG)、总胆固醇(CHOL)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)水平.结果 HFHD组治疗3个月后血磷、PTH、TG、LDL水平较治疗前下降,HDL水平较治疗前升高,差异均有统计学意义(均P<0.05);与HD组治疗后比较,血磷、PTH和TG明显下降,HDL升高,差异均有统计学意义(均P<0.05).HD组各项指标与治疗前的差异均无统计学意义(均P >0.05).结论 与常规透析相比,HFHD更能降低MHD患者血磷、PTH、TG和LDL,提高患者生存率.

Abstract:Objective To compare the effect of high flux hemodialysis(HFHD) with conventional hemodialysis (HD) for patients with maintenance hemodialysis(MHD).Methods Forty MHD patients were randomly divided into conventional hemodialysis group (HD group) and high flux hemodialysis group (HFHD group).Blood samples were collected at d1 of hemodialysis and 3 months after treatment,serum calcium,phosphate,parathyroid hormone (PTH),triglyceride (TG),total cholesterol (CHOL),high density lipoprotein(HDL) and low density lipoprotein(LDL) were measured.Results In group HFHD,serum phosphate,PTH,TG,LDL levels 3 months after treatment were decreased and HDL levels were increased than those before treatment (all P<0.05);and the serum phosphate,PTH and TG levels were lower and HDL levels were higher than those in HD group after treatment (P< 0.05).However,there were no significant differences of above indexes in group HD before and after treatment (P >0.05).Conclusion Compared with the conventional dialysis,HFHD can reduce serum phosphate,PTH,TG and LDL more significantly for MHD patients.
