非小细胞肺癌的外科治疗

目的：探讨手术治疗非小细胞肺癌（NSCLC）的临床疗效。方法：选择本科2005年4月～2010年1月收治的非小细胞肺癌患者230例,其中,肺叶切除204例、楔形切除15例、肺段切除5例、袖状切除4例、全肺切除2例,对其临床资料进行总结分析。结果：230例患者手术顺利,无一例发生围术期死亡,19例患者发生并发症,其中,肺部感染7例、左心衰3例、胸腔感染3例、肺泡胸膜瘘2例、呼吸衰竭、胸内出血、乳糜胸、支气管胸膜瘘各1例,平均住院时间为15.5d,5例患者死于并发症。结论：根据非小细胞癌患者疾病情况选择不同的外科手术方式治疗,可有效的清除病灶阻断淋巴转移,并且最大限度地保留了患者的肺功能,提高了其生活质量,临床疗效满意。

Surgical treatment of non-small cell lung cancer

Objective：To evaluate the clinical curative effect of non-small cell lung cancer by surgical treatment.Methods：Collected 230 cases in our abteilung who with non-small cell lung cancer from April 2005 to January 2010,204 cases did the regular lobectomy and 15 cases did the wedge resection and 5 cases did the segmentectomy resection,only 4 cases did the sleeve lobectomy and 2 cases did the total pneumonectomy,analyze the clinical data.Results：All the patients smoothly live through the operation,none death happened perioperative period,postoperative complication happened in 19 cases,7 cases happened pulmonary infection,3 cases happened left heart failure,chest infection in 3 cases,alveolipleura fistula in 2 cases and each of the case of respiratory failure,bleeding,chylothorax and broncho-pleura fistula,the everage inpatient time was 15.5 days,5 cases dead for complication.Conclusion：Select the different surgical treatment according to the comorbidities,not only cleare the focus validly and stope the lymphatic transfer,save the functional lung tissue,enhance the patients＇ quality of life,obtaine a satisfactory clinical effect.
