重度胸腹联合伤28例外科治疗分析

目的　总结重度胸腹联合伤的外科治疗的经验教训。方法　对我院 2 0 0 1年 10月— 2 0 0 4年 6月收治的 2 8例重度胸腹联合伤的治疗情况进行回顾分析。结果　存活 2 6例 ,死亡 2例 ( 1例死于ARDS ,1例死于肝静脉断裂出血 )。结论　早期尽快补充血容量抗休克很关键 ;有手术指征者应尽快手术 ,优先解决危及生命的创伤 ;手术切口经胸入路可以一处体表切口解决胸腹两处创伤的探查与修复

Therapeutic Analysis of Severe Thoracico-abdominal Combined Injures in 28 Cases

Objective To summarize the experiences and lessons from the surgical treatment of severe thoracico-abdominal combined injures (TCI).Methods A retrospective study was carried out in 28 cases of TCI surgically treated in our hospital from October, 2001 to June, 2004. Results Of 28 cases, 26 survived after surgical treatment, and two died due to neglect of the therapy of adult respiratory distress syndrome (ARDS) as well as bleeding of the ruptured hepatic vein. Conclusion It is crucial that anti-shock as quick as possible contribute to the compensation of the blood volume. The operations should be performed on those who have indications so that the vital wounds or injuries can be treated. The access should be made on the thoracic part, through which both of thoracic and abdominal injuries can be explores and then repaired.
