腹腔镜胆囊切除术与小切口胆囊切除术疗效的对比分析

目的探讨胆囊切除术适应证患者的理想术式。方法回顾45例腹腔镜胆囊切除术（LC）和45例小切口胆囊切除术（MC）患者的临床资料，从手术时间、术中出血量、术后肠功能恢复时间、术后住院天数、总费用、并发症等方面分析两者的优缺点。结果两组手术时间和术中出血量比较差异无统计学意义（P〉0．05），LC组术后肠功能恢复时间和术后住院天数少于MC组，但总费用明显高于MC组，差异有统计学意义（P〈0．05）。腹腔镜组术中胆总管损伤1例，术后并发膈下脓肿1例，剑突下戳孔感染1例；小切口组术后出现脂肪液化及伤口感染2例。结论两种术式各有优缺点，任何一种术式都不能适应所有胆囊切除适应证患者，选择何种术式要根据患者具体情况决定。

Comparative analysis of curative effect on laparoscopic cholecystectomy and small-incision cholecystectomy

Objective To discuss the ideal operation type of patients who had indication of cholecystectomy.Methods The clinical data of 45 patients with laparoscopic cholecystectomy (LC) and 45 patients with small-incision cholecystectomy (MC) were analyzed retrospectively.The advantages and disadvantages of both groups were analyzed from the operative time,blood loss,recovery time of postoperative intestinal function,postoperative hospital stay,total cost,complications,and so on.Results There were no statistically significant differences of operation time and blood loss during the operation between the two groups (P> 0.05).The recovery time of postoperative intestinal function and postoperative hospital stay in LC group were less than the MC group,but the total cost of which was significantly higher than the MC group,the differences was statistically significant (P< 0.05).In the LC group,there was 1 case of bile duct injury during the operation,1 case of complicated subphrenic abscess after the operation and 1 case of infection of puncture below the xiphoid process;in the MC group,there were 2 cases of fat liquefaction and wound infection.Conclusion Both operations have their own advantages and disadvantages.Any kind of operation types are not fit for all patients with cholecystectomy,the selection of operation types are decided by the individual conditions of patients.
