经皮肾镜取石术与经皮肾微造瘘取石术治疗肾结石的疗效比较

目的探讨并比较经皮肾镜取石术与经皮肾微造瘘取石术治疗肾结石的临床疗效。方法将86例肾结石患者随机分为PCNL组和MPCNL组,其中PCNL组肾结石患者采用经皮肾镜取石术治疗,而MPCNL组肾结石患者则采用经皮肾微造瘘取石术治疗,比较两组患者的手术时间、结石清除率及并发症发生情况。结果两组患者手术时间和结石清除率相比无明显差异（P〉0.05）。两组患者术后输血率、肾通道裂伤、冲洗液明显外渗及急性呼吸窘迫综合征等发生情况均无明显差异（P〉0.05）,但是MPCNL组的术后发热发生率明显高于PCNL组（χ2=4.02,P〈0.05）。MPCNL组的术后并发症的总发生率明显高于PCNL组（χ2=8.55,P〈0.05）。结论 PCNL与MPCNL均是治疗肾结石的有效方法,两种方法在手术时间和结石清除率方面均无明显差异,但是随着手术时间的延长,MPCNL的术后并发症发生率增加。

Compare the efficacy of percutaneous nephrolithotripsy and mini invasive percutaneous nephrolithotripsy in treatment of kidney stones

Objective To evaluate and compare the efficacy of percutaneous nephrolithotripsy and mini invasive percutaneous nephrolithotripsy in treatment of kidney stones.Methods Eighty six cases of kidney stones patients were randomly divided into PCNL group and MPCNL group.PCNL group was treated with percutaneous nephrolithotomy and MPCNL group was treated with mini invasive percutaneous nephrolithotripsy.Operative time,stone-free rate and complications were compared between two groups.Results The operative time,and stone clearance rate had no significant difference between two groups(P > 0.05).Postoperative blood transfusion rate,renal laceration channel,clear fluid extravasation and acute respiratory distress syndrome also had no significant difference between two groups(P > 0.05).But the incidence of fever in MPCNL group was significantly higher than PCNL group(χ2=4.02,P < 0.05).MPCNL group overall incidence of postoperative complications was significantly higher than PCNL group(χ2=8.55,P < 0.05).Conclusion PCNL and MPCNL were both the effective way in treating kidney stones.And two methods had no significantly different in operative time and stone clearance rates.But with the extension of operation time,MPCNL increases in the incidence of postoperative complications.
