中西医结合治疗重症急性胰腺炎的临床疗效观察

目的探讨中西医结合治疗重症急性胰腺炎（severe acute pancreatitis，SAP）的临床疗效。方法回顾性分析本院急诊科确诊的SAP患者74例的临床资料，分为观察组和对照组，每组37例。对照组给予禁食、补液、营养支持、抗感染等西医治疗，观察组在对照组基础上加用清胰汤治疗。观察两组症状缓解时间、住院时间、实验室指标、并发症及死亡情况，并进行比较。结果观察组腹痛、腹胀缓解时间、排便时间及住院时间均少于对照组（P〈0．05）。治疗3d后，观察组PAF、ET、TNF-α、IL-6、CRP水平均低于对照组（P〈0．05）。观察组死亡率低于对照组（P〈0．05）。结论中西医结合治疗重症急性胰腺炎疗效安全、有效，值得临床推广使用。

Therapeutic effect observation of combination of Chinese traditional and Western medicine in the treatment of severe acute pancreatitis

Objective To discuss the therapeutic effect of combination of Chinese traditional and Western medicine in the treatment of severe acute pancreatitis (SAP).Methods Clinical datas of 74 cases of patients who were diagnosed with SAP in emergency department of our hospital were analyzed retrospectively.All cases were divided into observation group and control group,each group had 37 cases.Control group was treated with ambrosia,fluid infusion,nutritional support,antiinfection and other treatment of western.Observation group was treated with Pancreatic clear soup on the basis of control group.Symptom remission time,hospital stays,laboratory parameters,complications and mortality were observed and compared between the two groups.Results Remission time of stomachache and abdominal distention,defecate time,hospital stays of the observation group were less than those of the control group,the differences were statistically significant (P<0.05).After treatment of 3 d,the level of PAF,ET,TNF-α,IL-6,CRP of the observation group were lower than those of the control group (P < 0.05).The mortality of the observation group was less than that of the control group,with statistically significant difference (P< 0.05).Conclusion The therapeutic effect of combination of Chinese traditional and Western medicine in the treatment of severe acute pancreatitis is safe and effective,which is worthy of clinical application.
