DHS与PFNA治疗骨质疏松性股骨粗隆间骨折疗效分析

摘    要:
目的采用DHS及PFNA治疗老年骨质疏松性不稳定性股骨粗隆间骨折,对比二者临床疗效。方法回顾性研究我院2010年1月至2011年1月52例采用DHS(22例)或PFNA(30例)治疗的骨质疏松性不稳定性股骨粗隆间骨折的老年患者,对比两组手术时间、术中出血量、骨折愈合时间、Harris评分及术后并发症等指标。结果 52例患者平均随访时间15.3月。两组骨折愈合时间及术后并发症无明显差异,但PFNA组相对于DHS组手术时间短、出血量少、髋关节功能恢复好。结论 PF-NA手术创伤小、出血量少、固定牢靠、并发症少,对老年骨质疏松性不稳定性股骨粗隆间骨折的治疗是一个较好的选择。

Abstract:
Objective To compare the curative effect of dynamic hip screw (DHS) and proximal femoral nail antirotation (PFNA) in the treatment of unstable osteoporotic intertrochanteric fracture in elderly patients. Methods 52 elderly patients with unstable osteoporotic intertrochanteric fracture treated with DHS (DHS group: 22 cases) or PFNA (PFNA group: 30 cases) in our hospital from January 2010 to January 2011 were analyzed retrospectively. The indices including operation time, intraoperative blood loss, fracture healing time, Harris scores and complications were compared in two groups. Results The mean follow-up time of 52 patients was 15.3 months. The fracture healing time and postoperative complication had no significant differences in two groups, while the PFNA groups had shorter operation time, less intraoperative blood loss and better recovery of hip function compared with DHS group. Conclusions PFNA internal fixation has less surgery trauma and bleeding, solid fixation and fewer complications, which is a better choice for the treatment of unstable osteoporotic intertrochanteric fracture in elderly patients.
