逆行股骨交锁髓内钉治疗股骨髁部骨折

目的；介绍并分析逆行股骨交锁髓内钉治疗股骨髁部骨折的方法及注意事项。方法：应用逆行股骨交锁髓内钉治疗股骨髁部骨折10例，按AO分类33A型6例，33C1型2例。33C2型2例。结果：随访平均9月（6-12月），10例患者平均骨性愈合时间16.8周（12-28周），达到骨愈合按Neer等的评分标准3例优秀（≥85分），6例好（≥70分），1例中（≥55分），没有差（≤55分）。结论：应用逆行股骨髓内钉治疗股骨髁部骨折具有高强度，低创伤，减少骨膜剥离和出血量，减少内外翻畸形发生，低感染率，利于早期活动关节，手术操作简便等优点，手术的关键在于复位时的轴向对线，短缩与旋转的纠正。

The Treatment of Femoral Condylar Fracture with the Retrograde Intramedullary Nail

Objective: To introduce the method of the treatment of femoral condylar fracture with the retrograde intramedullary nail. Methods: 10 cases of femoral condylar fractures were treated with the retrograde intramedullary nail. There were 6 AO/ASIF 33A, 2 AO/ASIF 33C1,2 AO/ASIF 33C2. Results: A shorter period follow-up was given. Mean 9 months(6-12 months), 10 cases union mean 16.8 weeks(12-28 weeks). Using the Neer scoring system, there were 3 excellent (85 or more points), 6 good (70 or more points), 1 fair (55 or more points), none of the cases was considered failure (less than 55 points). Conclusions:The use of the retrograde intramedullary nail offers some advantages: sufficient stiffness, decreased complications, provide with benefits of knee joint motion, no deep infaction, no knee problems, and decreased injuries of the soft-tissue. Intraoperative determination of alignment, avoiding shortening and torsion were the major difficulties.
