新生血管性青光眼的治疗策略

新生血管性青光眼(neovascular glaucoma,NVG)是一种难治性、继发性青光眼,病因复杂、多样.目前NVG常规的治疗方法效果多不理想,抗血管内皮生长因子(vascular endothelial growth factor,VEGF)药物的应用为抗青光眼手术提供了宝贵的时间窗.本文归纳了NVG的综合治疗策略,其治疗方法包括应用降眼压药物、眼内注射抗VEGF药物、单独抗青光眼手术或联合白内障摘出、玻璃体切除术以及在此基础上完成全视网膜光凝治疗等.本文对NVG的病因、发病机制、诊断及治疗进行总结,提出治疗NVG的核心目的是保留患者视功能,目标是完成全视网膜光凝,关键手段是抗VEGF及抗青光眼手术,其宗旨是最大限度挽救患者残存视功能.

The treatment of neovascular glaucoma

Neovascular glaucoma (NVG) is a refractory,secondary glaucoma.Its cause is complex and diverse.At present,NVG conventional treatment effect is not ideal,most to take temporary solution without a permanent cure.Anti-VEGF drug application is to provide valuable time window for anti-glaucoma surgery.This article proposed the establishment of comprehensive treatment of NVG strategy.The comprehensive treatment methods include the use of antihypertensive drugs,intraocular injection of anti-VEGF drugs,anti-glaucoma surgery alone or combined cataract extraction,vitrectomy and on the basis of completepanretinal photocoagulation treatment.The etiology,pathogenesis,diagnosis and treatment of NVG are summarized.The major purpose of treatment is to preserve the visual function of patients with NVG.The goal of treatment is to complete panretinal photoeoagulation.The key means of treatment are anti-VEGF and anti-glaucoma surgery in order to save the patient residual visual function.
