胆系恶性肿瘤MMP1和TIMP1表达及临床病理意义

研究胆囊癌和肝外胆管癌组织中MMP1和TIMP1表达特征及其临床病理意义。方法 :常规石蜡包埋切片行MMP1、TIMP1多克隆抗体ABC免疫组化法。结果 :4 0例胆囊癌MMP1和TIMP1阳性病例分别为 2 2例 (43% )和 2 4例 (6 0 % ) ,4 2例胆管癌则分别为 17例 (5 2 % )和 2 0例 (48% ) ;高分化腺癌和未转移胆系癌MMP1阳性率明显低于低分化腺癌或未分化癌和转移胆系癌 ,TIMP1表达阳性率则相反 ;MMP1在胆囊癌或胆管癌中表达与TIMP1表达呈密切负相关 ;MMP1阳性表达者生存期明显低于阴性表达者 ,但TIMP1则相反。结论 :MMP1和TIMP1表达可反映胆囊癌或胆管癌临床病理特征、生物学行为、转移发生及其预后 ,两者均为重要生物学标记物。

EXPRESSION OF MMP1 ,TIMP1 AND ITS CLINICOPATHOLOGICAL SIGNIFICANCE IN GALLBLADDER CARCINOMA AND EXTRAHEPATIC CHOLANGIOCARCINOMA

Objective:To determine the expression of MMP 1,TIMP 1 and its clinicopathological significance in gallbladder carcinoma and extrahepatic cholangiocarcmoma.Methods:The expressions of MMP 1 and TIMP 1 were detected by immunohistochemical method of avidin-biotin complex in formalin-fixed and routinely paraffin-embedded sections.Results:The positive case of MMP 1 and TIMP 1 was 17 cases (43%) and 24 ones (30%) in 40 cases with gallbladder carcinoma, and that was 22 cases (52%) and 20 ones (48%) in 42 cases with extrahepatic cholangiocarcinoma, respectively. The positive rate of MMP 1 was significantly lower in well-differentiated carcinomas and without metastasis than that in poorly-or un-differentiated carcinomas and with metastasis in gallbladder carcinoma or extrahepatic cholangiocarcinoma, while that of TIMP 1 was opposite. The close negative correlation was found between the expression of MMP 1 and TIMP 1 in gallbladder carcinoma or exhepatic cholangiocarcinoma. The survival over 1 year was significantly lower in the positive cases of MMP 1 than that in the negative ones, that of TIMP 1 was opposite. Conclusions:The expression of MMP 1 or TIMP 1 might closely relate to the clinicopathological feature, biological behaviors, metastasis and prognosis of gallbladder carcinoma and extrahepatic cholangiocarcinoma, and they might be the important biological markers.
