单侧髁状突骨折手术与非手术治疗的比较性观察

为评价髁状突骨折手术和非手术治疗的结果,采用标准化临床检查和下颌骨后前位X线片计算机模拟显示,分析20例手术治疗和54例非手术治疗的单侧髁状突骨折的解剖结构及功能恢复情况。临床检查结果显示,最大开口度、下颌偏移及前伸运动的改变在手术治疗组和非手术治疗组无明显差异,但放射检查显示,髁状突复位程度手术治疗组明显优于非手术治疗组。认为手术治疗和非手术治疗相比,可以比较准确的恢复髁状突的解剖结构,使面部形态及咬合关系得到较完整的恢复。

THE COMPARATIVE OBSERVATION OF THE SURGICAL TREATMENT AND CONSERVATIVE TREATMENTFOR UNILATERAL CONDYLAR PROCESS FRACTURE

To evaluate the therapeutic effects of surgical and conservative treatment for condylar process, the restorative conditions of anatomical structure and functions of unilateral condylar process of twenty cases treated with surgery and fifty four cases treated with conservative methods were analyzed by standard clinical examinations and by evaluated results of the computer simulated graphic presentations of posteroanterior radiographs of the mandible. The clinical examination results showed that there was no difference between the surgical group and the conservative group in maximal mouth opening , deviation and protrusion movement; while the results according to radiography showed that the reposition effects of condylar process in the surgical group was better than that in the conservative group, suggesting that compared with the conservative methods, surgical treatment can restore the anatomical structure of condylar process more accurately, which recovers the facial appearance and occlusion more completely.
