99例慢性阻塞性肺疾病中医证候横断面调查

目的探讨慢性阻塞性肺疾病(COPD)的中医症状、证候特征。方法制定COPD中医证候调查表,对99例患者进行横断面临床调查。结果急性加重期实证的发生率依次为:血瘀证、痰热蕴肺证、表寒肺热证、痰湿阻肺证;虚证的发生率依次为:肺气虚证、脾气虚证、肾阳虚证。缓解期实证的发生率依次为:血瘀证、痰热蕴肺证、痰湿阻肺证;虚证的发生率依次为:肺气虚证、脾气虚证、肾阳虚证。结论急性加重期是外邪诱发而病情急性加重的本虚标实、虚实夹杂、以实为主、病机复杂的时期。缓解期是以肺、脾、肾三脏气虚、阳虚为主,伴有血瘀、痰热等本虚标实,以虚为主的病期。

Investigation on TCM Syndromes of 99 Cases of Chronic Obstructive Pulmonary Diseases

Objective To study the characteristics of TCM symptom and syndrome of chronic obstructive pulmonary disease(COPD).Methods The TCM syndrome investigation tables on COPD were made to carry out the transection clinical investigation on 99 cases.Results In the acute aggravating stage,the incidence of excess syndrome was in an order of blood stasis syndrome,phlegm heat in lung syndrome,exterior cold and lung heat syndrome,and phlegm damp in lung syndrome;while the incidence of deficiency syndrome was in an order of lung-qi deficiency syndrome,spleen-qi deficiency syndrome,and kidney-yang deficiency syndrome.In the relieved stage,the incidence of excess syndrome was in an order of blood stasis syndrome,phlegm heat in lung syndrome,and phlegm damp in lung syndrome;while the incidence of deficiency syndrome was in an order of lung-qi deficiency syndrome,spleen-qi deficiency syndrome,and kidney yang deficiency syndrome.Conclusion The acute aggravating stage is characterized by deficiency in orgin and excess in superficiality,complicated case of deficiency with excess,induced by exogenous pathogenic factors.The relieved stage is the one in which the pathological changes are symptomized as qi deficiency of lung,spleen and kidney,mainly yang deficiency,and accompanied with manifestations of excess in superficiality,blood stasis and phlegm heat,mainly deficiency.
