经皮椎体后凸成形术治疗脊柱骨质疏松性压缩骨折的疗效和安全性观察

目的 探讨采用经皮椎体后凸成形术(PKP)治疗脊柱骨质疏松性压缩骨折的临床效果及安全性.方法 将2009年5月至2011年5月收治的脊柱骨质疏松性压缩骨折患者40例随机分为两组(PKP组和PVP组),每组各20例,分别采用PKP及经皮椎体成形术(PVP)治疗,分别记录两组手术前后责任椎高度恢复情况,活动能力评分、镇痛药评分、视觉模拟评分(VAS)及术后并发症情况.结果 两组患者术后复查,均基本恢复椎体高度,但观察组明显优于对照组(P<0.05).两组术后活动能力评分、镇痛药评分、VAS评分较术前均有改善(P<0.05),但两组差异不明显(P>0.05).观察组无并发症发生,对照组骨水泥渗漏3例,两组均无脊髓及神经损伤.结论 PKP能有效地治疗骨质疏松脊柱压缩骨折,且具有良好的安全性.

Study on efficacy and safety of percutaneous kyphoplasty in treatment of osteoporotic vertebral compression fracture

Objective To explore the clinical efficacy and safety of percutaneous kyphoplasty （PKP） in treatment of patients with osteo- porntic vertebral compression fracture. Methods Forty cases of osteoporotic vertebral compression fracture during May 2009 to May 2011 were treated by PKP （PKP group, n = 20 cases） and percutaneous vertebroplasty （PVP） （PVP group, n = 20 cases） , height of vertebrae, activity scores, analgesic scores, VAS scores and complications were recorded during preoperation and postoperation periods. Results Increase of verte- bral height in PKP group was better than that of PVP group （ P 〈 0.05 ）. The activity scores, analgesic scores and VAS scores were obviously im- proved after operation in these two groups （ P 〈 0.05 ）, but there was no significant difference between these two groups （ P 〉 0.05 ）. There was no complication occurred in patients of PKP group but cement leakage had been seen in 3 cases of PVP group, and there was no spinal cord and nerve damage occurred in these two groups. Conclusion PKP is safe and effective in treatment of patients with osteoporotic vertebral compression fracture.
