重症肌无力病人淋巴细胞对AchR的反应性和血清抗AchR抗体关系的研究

报告92例重症肌无力病者109次TEEM和抗AchR抗体的配对检测结果,发现延髓肌、脊髓肌型患者两种检测异常者均较高,眼肌型和全身肌无力型患者无显著差异。30岁以下发病者的细胞免疫异常显著高于30岁后发病者,抗AchR抗体的阳性检出率与年龄无明显相关。认为重症肌无力的免疫治疗应视发病年龄而別,30岁以下的女性患者应抑制细胞免疫反应,30岁以后者应以抑制体液免疫的方法为首选。

STUDY ON RELATIONSHIP BETWEEN THE IMMUNORESPONSE TO ACHR ON LYMPHOCYTES AND SERUM ANTI-ACHR ANTIBODIES IN MYASTHENIA GRAVIS

109 times of paired measurements of TEEM(Tanned erythrocytes electrophoretic mobility test)and AchR antibodies(acetylcholine receptors antibodies)were observed in 92 cases of myasthenia gravis.The results showed that the positive rate of AchR antibodies was significantly higher(P<0.001)than that of TEEM in whole MG group.The positive rate of TEEM in female patients with onset before age 30 was significantly higher than that of those with onset after 30.In contrast,the positive rate of AchB,antibodies in female patients with onset before age 30 was less than that those with onset after 30(P<0.05).We suggest that female patients with onset before age 30 have more sensitive cellular immune response to AchB,those with onset after 30 have more humoral immune response to AchB.We would consider the pattern of immune response AchB to be an important supplementary parameter in the treatment of myasthenia gravis.
