腰椎后关节退变与腰痛

本文回顾性研究了４２例腰椎间盘摘除手术病人（其中３２列有腰痛病史）腰椎后关节的病理组织学和Ｘ线照片，探讨腰椎后关节退变与腰痛的关系。研究结果表明有腰痛史患者腰椎后关节退变率（８７．５％）显著高于无腰痛史患者（２０％）（Ｐ＜０．０００１）。作者认为腰椎后关节主要功能是控制和稳定扭转力，当关节面应力不平衡时至使关节软骨营养发生障碍而产生退变。由于腰背部感觉受脊神经根后支支配，后关节组成椎间孔管的后壁。关节突骨赘或上关节突位置前移，脊神经根受压，可引起腰痛。临床上有些腰痛患者应重视腰椎后关节退变因素。

Degeneration of Posterior Lumbar Joints and Backpain

retrospective study of the hi,topathology and arthrograms of posterior lurnbar joints on 42 patients with lumbar inter-vertbral disk extraction operations is made in this paper(32 patients with history of backpain).The relationship between pos-terior lumbar joints and backpain is investigated.The result revealed that the degenerate rate of the posterior lumbar jointsin patients with backpain(87.5%)was remarkably higher than that of the non-backpain patients(20%)(p<0.0001). The authtor believe that the main functions of posterior lumbar joints are controlling and stabilizing the articular force. Mal-nutrition of the artitcular facet would be happened as the stress is out of balance and consequent degeneration of the lumbarjoints. The dorsal branch of the spinal nerve root acts as the sensory nerve on the lumbar region. Posterio,lumbar jointscomlposed of the posterior wall of the intervertebral foramen canal.The formation of zygapophysial osteophyte or the frontshift of the superior articular process may press upon the spinal nerve root and caused backpain.Therefore one should paygreat attention to the degeneration of posterior lumbar joints as an important factor in clinical backpain patients.
