HIV相关肺疾病的临床分析

目的提高对HIV相关肺疾病的认识，减少临床漏诊及误诊。方法对某市中心医院近5年来确诊的34例HIV患者进行回顾性分析。收集其临床资料，统计相关肺疾病的发生率，并比较有／无相关肺疾病患者的病死率有无差异。结果本组HIV患者中以男性中青年多发，占82．35％；相关肺疾病的发病率高，为67．65％，以急性感染为主；出现相关肺疾病组患者的病死率明显高于无肺疾病患者（26．09％掷18．18％，P=1．000）。临床以发热、咳嗽及气促、呼吸困难为主要表现。结论对既往身体健康的中青年患者，若出现反复发热、咳嗽、气促甚至呼吸衰竭、肺动脉高压应及时检查HIV抗体，减少HIV漏诊。

Clinical analysis of HIV-associated pulmonary diseases

Objective To improve the understanding of the HIV-associated pulmonary diseases for avoiding misdiagnosis and missed diagnosis. Methods The data of 34 cases with HIV making a definite diagnosis in the past five years were collected, and the incidence and mortality of the HIV-assoeiated pulmonary disease were analyzed retrospectively. Results The man of middle and young age was frequently in the group（82.35% ）. The incidence of the HIV-associated pulmonary disease was 67.65%. The acute infection was more common. The mortality with the HIV-asso- ciated pulmonary disease was higher than without（26.09% vs 18.18% ,P = 1. 000 ）. The main clinical performance were fever, cough, shortness of breath and difficulty in breathing. Conclusion If the man of middle and young age who was healthy in past has recurrent fever, cough, shortness of breath even respiratory failure and pulmonary arterial hypertension , the antibody of HIV should be examined in time to avoid misdiagnosis the HIV-associated pulmonary diseases.
