白内障超声乳化术治疗闭角型青光眼的临床研究

摘    要:目的观察白内障超声乳化吸除术治疗闭角型青光眼的临床效果。方法对48例52眼急性闭角型青光眼临床前期、先兆期或缓解期,以及慢性闭角型青光眼合并白内障患者,单纯施行白内障超声乳化联合人工晶体植入的手术,观察手术前后的房角、眼压以及视野变化,随访6~28个月,平均(15.4±6.7)个月。结果术后随访眼压为(12.34±3.76)mmHg,比术前用药后眼压(19.24±5.91)mmHg明显降低(P〈0.01)。前房角镜检查:术前房角关闭粘连〈1/4圆周者20眼,1/4—3/4圆周者19眼,≥3/4圆周者13眼。术后房角均有增宽,关闭粘连〈1/4圆周者31眼,1/4~3/4圆周者14眼,≥3/4圆周者7眼。视野明显好转,视敏感度均增加。结论对于合并有白内障的闭角型青光眼患者施行单纯的超声乳化手术可以有效开放前房角及控制眼压。

Abstract:Objective To evaluate the clinic effects of phacoemulsification in early primary angleclosure glaucoma with cataract. Methods 48 cases ( 52 eyes ) with early primary angle-closure glaucoma and cataract were received phacoemulsification. The intraocular pressure ( IOP ), anterior chamber angle and visual acuity were examined before and after operation. All were followed up 6 to 28 months. Results The mean postoperative IOP ( 12.34±3.76 mmHg, 1 mmHg=0.133kpa ) was lower than the mean preoperative IOP ( 19.24±3.76 mmHg ). The closed anterior chamber angles were opened, the visual acuity improved significantly. Conclusions In cases of early primary angle-closure glaucoma with cataract, the anterior chamber angle became wider after phacoemulsification. The visual acuity improved and IOP can be controlled after the operation.
