腰椎间盘突出症再手术的原因

目的探讨腰椎间盘摘除术后疗效欠佳及再手术的原因。方法对３１例腰椎间盘突出症术后复发者进行再手术，对术中所见及随访结果进行分析。结果 随访６个月－７年，术后优良率８０．６％。结论再手术原因主要有：（１）诊断不明或漏诊，尤其是特殊类型的腰椎间盘突出。（２）手术操作不够精细及手术适应证掌握不严。（３）术后腰椎不稳。（４）术后处理不当。严格掌握手术指征可收到良好疗效。

Causes of re operation on the herniation of lumbar disc

Objective To investigate the reasons of re operation and unsatisfied effect after the extirpation of herniation of lumbar disc. Methods 31 relapse patients of the herniation of lumbar disc were operated on and what was found in the operation was analyzed and the follow up survey conducted. Results Follow up of 6 months to 7 years was carried out, the excellent and good rate of post operation was 80 6%. Conclusions The main causes of re operation are as follows: (1) unclear diagnosis and mis diagnosis, especially the special type of herniation of lumbar disc; (2) uncareful performance and problems of choosing indications; (3) instability of lumbar vertebrae after operation; (4) unsuitable treatment after operation. Satisfactory effect can be obtained through strict selection of operation indications.
