颈前路手术治疗多节段颈椎间盘突出症

目的：评价颈前路手术治疗多节段颈椎间盘突出的术后疗效,手术并发症及防治措施。方法：对多节段颈椎间盘突出症患者经颈前路手术患者35例（随访33例）,平均随访18月。35例患者中15例行多节段颈椎间盘摘除,椎间植骨融合,颈前路接骨板内固定术;20例行椎体次全切除,长条植骨颈前路接骨板内固定术。获得随访患者予JOA评分,比较术后JOA评分改善情况及手术前后的颈椎MR,比较其脊髓矢状径以评价术后疗效。结果：35例患者中,失访2例,33例患者得到最终随访。33例术前JOA评分2～14分,平均8.88±0.64分,术后最终随访时8～16分,平均14.1±0.39分,两者有显著差异（P〈0.01）。其中优良13例（40%）,好转21例（64%）,无效1例（3%）,加重1例（3%）。术后改善率10%～93%,平均61%。术前脊髓矢状径为3.3～8.1 mm（5.2±1.1mm）,术后脊髓矢状径为7.4～13.4 mm（9.7±1.7mm）,P〈0.01。结论：颈前路手术效果明显,是治疗多节段颈椎病的首选方案。

Study on Treating Multilevel Cervical Disc Herniation by Cervical Anterior Operation
