彩色超声诊断胆囊占位性病变的临床价值

目的探讨彩色超声对胆囊占位的良恶性病变的诊断价值。方法分析经手术病理证实的116例胆囊占位性病变的声像图和彩色血流图。结果胆囊良性占位内血流以点状、线状为主，阻力指数（RI）=0．52±008，明显低于恶性组（RI=0．78±0．12），P〈0．05；胆囊恶性占位内血流以分枝状或杂乱型为主，最大血流速度（vmax）=（433±18．3）cm／s，明显高于良性组，p〈0．05。结论彩超对胆囊占住性病变的鉴别诊断有较高的价值，可避免不必要的手术，为临床提供重要的参考价值。对于一些具有交叉重叠的声像图需加以临床综合分析，以提高诊断准确率。

Diagnostic value of ultrasonography in gallbladder space-occupying leisions

Objective To investigate the diagnostic value of color ultrasonography in gallbladder neoplasms. Methods Ultra- sonogram and color blood flow of 116 cases of gallbladder neoplasms confirmed with pathology were analyzed. Results Dot- like or linear blood flow was commonly presented in benign space occupying lesion, and the resistance index （R1）was 0.52 ：t： 0.08, which was significantly lower than that of the malignant group （RI=0.78 ±0.12）, P 〈 0.05. Dendroid or confused blood flow was commonly presented in malignant neoplasms, and the peak veloity was （43.3 ± 18.3 ）cm/s, which was significantly higher than that of the benign group, P 〈 0.05. Conclusion Color ultrasonography is very useful in the differential diagnosis of gallbladder disease. R is helpful to avoid unnecessary surgery and provide an important reference for clinic. It is necessary to analyze comprehensively for overlapping ultrasonogram in order to to improve its diagnostic accuracy.
