胃癌多基因表达及病理

作者应用免疫组化技术检测７５例胃癌组织中多种基因表达的变化，分析它们与胃癌病理特征及预后的关系。结果：１．Ｐ５３和ｃ－ｅｒｂＢ－２表达分别主要出现于胃粘膜癌变早期和晚期，前者在肠型胃癌中呈高表达，后者在高分化胃癌中阳性率较高；ＥＧＦＲ和ｒａｓ表达与胃癌浸润转移指标呈正相关；２．ｒａｓ表达与ＥＧＦＲ表达呈正相关，而与ｃ－ｅｒｂＢ－２呈负相关；３．对６５例进展期胃癌作生存分析，ｒａｓ、ＥＧＦＲ表达和ＤＮＡ倍体类型及浸润转移指标对胃癌预后有显著影响；Ｃｏｘ模型多因素分析，ＤＮＡ倍体、ＥＧＦＲ表达、淋巴结转移和远处转移是影响胃癌预后的独立因素。结论：多基因表达的检测对胃癌的诊断、恶性程度的判断及预后的估计有一定的意义。

Clinicopathological and Prognostic Significance of Multiple Molecular Genetic Abnormalities in Gastric Cancer

By means of immunohistological technicque,the expressions of P53,c-erbB-2,EGFR and ras were examined on the adjacent sections of gastric cancer(GC),and their clinicopathological and prognostic significance were studied.Results:1.0ver expressions of P53 and c-erbB-2 were the early and late sign of malignant change of gastric mucosa, respectively;EGFR and ras had closely association with the degree of malignancy of GC.2.The expression of ras had positive relationship with the expression of EGFR,and negative relationship with c-erbB-2.3.The multivariate analysis of Cox model of GC showed only DNA ploidy,expression of EGFR,lymph node metastasis and distant metastasis were independent prognostic factors of GC.
