左布比卡因与布比卡因腰麻用于剖宫产术的比较

目的比较0．5％等比重左旋布比卡因与布比卡因腰麻用于剖宫产术的麻醉效果。方法选择ASA I～Ⅱ级于腰麻下行择期剖宫产术的患者60例,随机均分为I组和Ⅱ组,分别蛛网膜下腔注射0．5％等比重左旋布比卡因和布比卡因2．4ml。观察麻醉起效时间、运动和感觉阻滞维持时间、新生儿Apgar评分和麻醉并发症。结果麻醉的起效时间I组显著晚于Ⅱ组(P<0．05);胸椎最高阻滞平面I组显著低于Ⅱ组(P<0．05);两组麻醉后血压显著低于麻醉前(P<0．05),但组间差异无统计学意义(P>0．05);术中运动阻滞级别、VAS评分、新生儿Apgar评分以及术后运动和感觉消退时间两组比较差异无统计学意义(P>0．05)。结论0．5％等比重左旋布比卡因起效比布比卡因慢,麻醉平面相对较低,更加安全。

Comparison of isobaric levobupivacaine with isobaric bupivacaine for lumbar anesthesia in caesarean section

Objective To compare the effects of isobaric levobupivacaine with isobaric bupivacaine for spinal anaesthesia in cesarean section. Methods Sixty ASA I - II patients undergoing cesarean section randomly divided into group I and group II . Two groups were injected 0.5 % isobaric levobupivacaine 2.4 ml and 0.5 % isobaric bupivacaine 2.4 ml into subarachnoid space, respectively .The onset time of anesthesia, the level of sensory block, the duration of anesthesia, motor blockade Bromage scales, intraoperative VAS scores, neonatal Apgar scores and anesthetic complications were observed. Results The onset time of anesthesia in group I was significantly longer than that in group II (P < 0.05 ), the highest level of block in group I was significantly lower than that in group II (P < 0.05) .The blood pressure decreased significantly after anesthesia in both groups (P < 0.05), but there was no difference between two groups (P >0.05). There were no difference between two groups in Bromage scales, intraoperative VAS scores, duration of anesthesia and neonatal Apgar scores (P > 0.05). Conclusion 0.5% isobaric levobupivacaine onsets slowlier and its highest level of block is lower than 0.5% isobaric bupivacaine for spinal anesthesia in cesarean section, therefore, it is safer than 0.5% isobaric bupivacaine.
