12导联动态心电图同步记录诊断变异性心绞痛的临床应用价值

目的：探讨12导联动态心电图同步记录诊断变异性心绞痛的临床应用价值。方法：所有患者均行常规和动态心电图检查，常规心电图采用单导心电图机，行12导联心电图检查；动态心电图采用12导联同步记录，分别分析ST抬高伴对应导联ST段下移、T波高尖伴对应导联T波倒置或低平、伴发的室性心律失常、U波倒置和房室传导阻滞等情况。结果：60例变异性心绞痛患者其常规心电图显示：ST段抬高0．1～0．2mV且伴对应导联ST段下移1例（1．67％）；动态心电图显示：ST段抬高0．1～0．2mV且伴有对应导联ST段下移18例（30．0％），且ST—T改变发生的时间为：第一高峰在00：00～08：00，第二高峰在13：00～15：00。结论：12导联动态心电图同步记录对于诊断变异性心绞痛具有重要的临床应用价值。

The Clinical Value of 12-lead Holter Monitoring for the Diagnosis of Variant Angina

Objective： To explore the clinical value of the 12-lead Halter monitoring for the diagnosis of variant angina. Methods： All patients underwent conventional single-lead electrocardiogram （ECG） recorded non-synchronously and 12-lead Holter monitoring recorded simultaneously. It were respectively analyzed of ST elevation with the corresponding ST-segment depression, T-wave height and tip with the corresponding T-wave inversion or low, ventricular arrhythmia, U-wave inversion and atrioventricular block. Results： There were totally 60 cases of variant angina patients. The conventional single - lead ECG showed the ST - segment elevation 0. 1 - 0. 2mV accompanied by the corresponding ST-segment depression in one case（ 1.67% ） but the 12-lead Holter monitoring showed it in 18 cases （30.0%）. The ST-T changes in Hoher monitoring occurred at a time as follows： the first peak at 00：00 -08：00, the second peak at 13：00 - 15：00. Conclusions ： 12-lead Hoher monitoring for the diagnosis of variant angina has important clinical value.
