铜绿假单胞菌氨基糖甙类药物产酶耐药机制的研究 |

摘 要: | 目的了解我院临床分离的30株多重耐药铜绿假单胞菌中16SrRNA甲基化酶基因及氨基糖甙类修饰酶(AMEs)基因存在状况。方法收集山东和江苏两地医院临床分离的铜绿假单胞菌共50株(山东烟台多重耐药30株,江苏泛耐株20株),采用聚合酶链反应(PCR)及序列分析的方法分析6种16SrRNA甲基化酶(armA、rmtA、rmtB、rmtC、rmtD、npmA)和6种AMEs基因aac(3)-I、aac(3)-II、aac(6’)-Ib、aac(6’)-II、ant(3”)-I和ant(2”)-I。结果山东烟台30株多重耐药铜绿假单胞菌中4种基因aac(3)-II、aac(C)-II、ant(3”)-I和ant(2”)-I的阳性株数分别为6株(20%)、14株(46.7%)、11株(36.7%)和1株(3.3%),其余8株基因均为阴性;AMEs基因总阳性率为50%(15/30)。江苏20株泛耐株中6种基因rmtB、aac(3)-II、aac(6’)-Ib、aac(6’)-II、ant(3”)-I和ant(2”)-I的阳性株数分别为ll株(55%)、12株(60%)、7株(35%)、12株(60%)、10株(50%)和9株(45%),其余6株基因均为阴性;AMEs基因总阳性率为95%(19/20)。结论铜绿假单胞菌存在16SrRNA甲基化酶基因为国内首次报道;铜绿假单胞菌AMEs基因携带率很高;两家医院检出酶基因种类有差别。

Abstract: | Objective To investigate the 16SrRNA methylases gene and AMES of 30 strains of multi-resistant pseudomonas aeruginosa isolatted from clinic work in our hospital. Methods Collected 50 strains of pseudomonas aeruginosa from two hospitals (30 strains multi-resistant pseudomonas aeruginosa from Yantai, Shandong, 20 swains pan-drugresistant from Jiangsu), and analysized 6 kinds of 16SrRNA methylases (armA, rmtA, rmtB, rmtC, rmtD, npmA) and 6 kinds of AMEs gene aac(3)-I, aac(3)-Ⅱ, aac(6')- I b, aac(6')- II, aac(3")- I, and aac(2")- I by PCR and sequence analysis. Results In 30 strains of multi-resistant pseudomonas aeruginosa from Yantai, the positive rate of 4 kinds of genes aac(3)-Ⅱ, aac(6')-II, aac(3")-I, and aac(2")-I were 20%(6/30), 46.7%(14/30), 36.7% (11/30) and 3.3% (1/30) separately .The 8 kinds of the rest genes were all tested negative.The total positive rate of AMEs gene was 46.7% (14/30). In 20 strains of pan-drug resistant from Jiangsu, the positive rate of 4 kinds of genes aac(3)- II, aac(6')-II, aac(3")- I, and aac(2")- I were 55% (11/20), 60% (12/20), 35% (7/20) and 60% (12/20), 50% (10/20), 45% (9/20) separately. The 6 kinds of the rest genes were all tested negative. The total positive rate of AMEs gene was 95%(19/20). Conclusions It is the first report that 16SrRNA methylases gene existed in pseudamonas aeruginosa; There was very high positive rate of AMEs genotypes in pseudamonas aeruginosa; There was different gene from two hospitals. |
