磁共振动态增强扫描及加权成像结合钼靶X射线成像对乳腺癌的诊断价值

摘    要:
目的:探讨MRI动态增强扫描(CE-MRI)、弥散加权成像(DWI)联合数字化钼靶X射线成像对乳腺癌的诊断价值。方法:搜集经手术病理证实或穿刺活检证实的32例乳腺癌病例,应用数字化乳腺机行乳腺内外侧斜位、头尾位摄片。采用Siemens1.5T超导磁共振扫描仪,乳腺专用相控阵线圈,检查序列T1WI、T2WI的压脂、DWI及VIBE动态增强扫描。结果:32例乳腺癌病例中30例DWI呈高信号,敏感度为93.8%(30/32),VIBE动态增强扫描,28例病变强化明显,结构清楚,明确诊断,敏感度为87.5%(28/32),钼靶X射线片发现确切病灶并诊断正确23例,敏感度为71.9%(23/32)。结论:MRI动态增强扫描、MRI扩散加权成像、数字化钼靶X射线成像三者联合应用对乳腺癌的检出率及诊断准确性有一定的提高。

Value of dynamic-contrast MRI in combination with diffuse weighted imaging and digital mammography for diagnosis of breast cancer

Abstract:
Objective: To investigate the value of contrast-enhanced MRI in combination with diffusion-weighted imaging(DWI) and digital mammography in differential diagnosis of breast lesions. Methods: A total of 52 cases of breast cancer confirmed by pathology or biopsy were enrolled in this study. Oblique view of the application of digital mammography inside and outside the line of breast, head and tail radiograph were obtained by using Siemens 1.5T superconducting magnetic resonance scanner. Breast dedicated phased array coil, the serial T1WI, T2WI, fat suppression, DWI and the VIBE dynamic enhanced scanning were used. Results: Of the 32 cases, 30 showed high signal in DWI, The sensitivity was 93.8%(30/32). As for the VIBE dynamic growth, 28 cases had marked enhancement and clear structure. The sensitivity was 87.5%(28/32). For mammography X-ray, the exact lesion was found and correctly diagnosed in 23 cases. The sensitivity was 71.9%(23/32). Conclusion: MRI dynamic contrast enhanced CT in combination with MRI diffusion-weighted imaging and digital mammography X-ray imaging can promote the detection rate and diagnostic accuracy of the breast cancer.
