老年不稳定型股骨转子间骨折髋螺钉固定与股骨头置换疗效对比

目的：比较人工股骨头置换术与动力髋螺钉（DHS）内固定治疗老年股骨转子间骨不稳定型骨折的临床疗效。方法：随机抽取2001年8月～2009年1月收治的老年不稳定型股骨转子间骨折56例,随机分为两组各28例,分别作DHS内固定和人工股骨头置换术。并进行回顾性分析评估这2种疗效并进行比较。结果：患者均获得随访,人工股骨头置换术的疗效显著,与DHS内固定组比较有显著性差异（P〈0.05）。结论：人工股骨头置换术治疗老年不稳定型转子间骨折与DHS内固定相比根据统计数据表明,具有手术时间短,出血量小,能早日下地负重锻炼,尽可能地使患髋关节功能恢复至受伤前的水平,大幅度降低各种并发症的发生等优点。

Effects comparison of dynamic hip screw (DHS) and artificial femoral head replacement on senile unstable intertrochanteric fracture

Objective: To compare the effects of dynamic hip screw(DHS) and artificial femoral head replacement on senile unstable intertrochanteric fracture.Methods: 56 senile unstable intertrochanteric fracture cases were randomly chosen and divided into 2 groups by half.One group was treated by DHS,the other by artificial femoral head replacement and the curative effects were compared.Results: According to the follow-up information,the artificial femoral head replacement showed better effects than that of the DHS group with significant differences(P<0.005).Conclusion: With advantages of shorter operation time,less haemorrhage volume and earlier weight-bearing exercises,artificial femoral head replacement can help patients regain hip joint function and reduce the complication incidence as much as possible.
