胸骨肿瘤CT诊断

目的：分析胸骨肿瘤CT表现，探讨CT诊断胸骨肿瘤价值。材料与方法：回顾分析1993年8月-2000年5月11例经手术或病理证实的胸骨肿瘤CT表现特点。结果：胸骨原发肿瘤6例，软骨肉瘤4例。软骨瘤，骨软骨瘤各1例；转移瘤5例，分别来源于肺癌2例，甲状腺癌，胃癌，恶性胸腺瘤各1例。转移瘤表现为溶骨性骨质破坏，可突破骨皮质侵犯周围组织；软骨瘤，骨软骨瘤均呈膨胀性生长，并见不同形式钙化或骨化，可压迫推移邻近器官；软骨肉瘤则为膨胀性骨质破坏，亦见各种钙化，可破坏骨皮质形成软组织肿块。结论：胸骨肿瘤CT扫描可明确病变部位，范围及其特点，结合临床初步判断肿瘤良恶性，为术前诊断及制定治疗方案提供帮助，但准确判断肿瘤良恶性有一定限度。

CT diagnosis of sternum tumor

Objective:To evaluate CT scan in the diagnosis of the sternum tumor.Materials and Methods:CT findings of sternal tumor in11pathologically proved cases were analyzed retrospectively.Re sults:Six cases of primary sternum tumors included4chondrosarcoma,1chondroma and1osteochondroma;five cases were metastatic tumor of the sternum(two from lung,and one each from thyroid,stomach and kidney).Primary sternal tumors appeared to be slow-growing,hard,fixed,and nontender masses,on CT the tumors exhibited a scalloped or lobulated contour,calcifications were detected on CT in all of the6patients,metastatic tumors exhibited osteolytic destruction of bone with soft tissue masses.Conclusion:CT scanning of sternum has the ability to detect subtle matrix mineralization and delineate the extent and probale origin of the tumor,CT strongly favored the preoperative diagnosis of primary sternal tumor and was helpful in surgical management.The limitation is the differentiation between benign and malignant lesions which may be difficult in cartilaginous tumors.
