代谢综合征患者凝血纤溶系统功能的变化

摘    要:目的探讨代谢综合征(MS)患者凝血纤溶系统功能的改变及其特点。方法对240例患者进行横断面研究,根据代谢综合征定义将患者分为MS组与非MS组,比较2组患者身高、体质量、腰围、血脂等生化指标,未诊糖尿病的进行葡萄糖耐量试验。测定2组患者的凝血酶时间(TT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白(Fib)、抗凝血酶III(AT-III)、D-二聚体、组织纤溶酶原激活物(t-PA)、I型纤溶酶原激活物抑制因子(PAI-1)、P-选择素、C-反应蛋白(CRP)等凝血纤溶系统功能指标,并进行比较。结果2组总胆固醇(TC)、低密度脂蛋白(LDL-C)、载脂蛋白(Apo)比较,差异均无统计学意义(P>0.05)。2组体质量指数(BMI)、腰围、收缩压、舒张压、三酰甘油(TG)、高密度脂蛋白(HDL-C)、空腹血糖、CRP、Lp(a)比较,差异有统计学意义(P<0.05);2组CRP、AT-Ш、D-二聚体、t-PA、PAI-1及P-选择素比较,差异有统计学意义(P<0.05)。结论MS患者存在凝血纤溶系统功能的异常。

Abstract:Objective To explore the characteristic of hemoglutination and fibrinolytic system function in patients with metabolic syndrome (MS). Methods A cross section study were carried out in 240 patients. According to definition of metabolic syndrome, the patients were divided as MS group and non MS group. The height, weight, waistline, blood lipids and other biochemical indicators were compared in two groups. The glucose tolerance experiment was applied in patients without diabetes. The indexes of bemoglutination and fibrinolytic system were detected in both groups,included TT, PT, APTF, Fib, AT-III, D-dimmer, t-PA, P-selectin, CRP et al. Results There was no differenees in gender and age between MS and non MS groups ( P 〉 0.05 ) , and no statistical difference in TC, LDL-C and Apo( P 〉 0.05 ). While in other indexes, included the BMI, waistline, SBP, DBP, TG, HDL-C, FBG, CRP, and Lp (a) ,there were statistical significance between two groups ( P 〈 0.05 ). In MS group, the CRP, AT-III, D-dimmer, t-PA, PAI-1 and P-seleetin had significant difference compared to non MS gourp ( P 〈 0.05 ). Conclusion It proved that the function of hemoglutination and fibrinolytic system was extraordinary in MS patients.
